Accident and Incident Investigations

Accident and incident investigation is used to determine the cause(s) of an accident and identify any failures of the health and safety management system. It allows measures to be put in place to eliminate or manage the activity to prevent re-occurrence

QSS can undertake most types of investigation, specialising in railway industry requirements. The process involves:

Inspection of the location and recording of evidence
Inspection of associated site health and safety documentation
Taking witness statements
Interviewing injured parties
Preparation of a report (Level1-3) indicating the immediate and underlying causes of the event and making recommendations as required
