Accident Investigation Training Courses

The QSS Group Limited are currently delivering an extensive program of training courses for Network Rail on the subject of Local and Formal Accident Investigation.

Due to the positive feedback received from candidates and Network Rail, other rail organisations have shown interest and commissioned QSS to design and deliver “bespoke” accident investigation courses not only based on national standards, but taking into account their own internal systems, procedures and more importantly risks.

On completion of the course, candidates will be able to demonstrate competence as an investigator at Local and Formal Level and have an understanding of the overall investigation process as detailed in the following:

  • Railway Group Standard GO/RT3473 – Accident and Incident Investigation – January 2007
  • Organisations Internal Procedures for Formal and Local Investigations into Safety Events

The QSS approach utilises a dedicated trainer to manage this process, who will interface personally with organisations to reach a clear understanding of their requirements. The dedicated trainer will then complete the course in full.

The “Lecture Style” modules are interactive and will populate candidates' thought processes when carrying out investigations. These sessions include:

  • Human error and human failure
  • Immediate and underlying causes
  • The national and internal investigation process (Draft, Local, Formal & HQ Led)
  • Critically assessing a preliminary report
  • Formulating action plans and making recommendations
  • Fact finding and evidence gathering
  • Interview techniques
  • Guidance on internal templates

The simulation and test sessions will include:

  • Short introductory questionnaire
  • Analysis of error types in relation to human failure
  • Recommendation Assessment

QSS will supply all training materials including:

  • Candidate Workbooks
  • Candidate Assessment Material
  • Course Scenario Material
  • Course Attendance Certificates

Course Duration

This is a two-day course.
