Accident and Incident Investigation Why do we need it? All organisations have a duty to establish why accidents and incidents occur in the workplace and should apply mitigating action to prevent reoccurrence. Who says so? The Health and Safety at Work Act and the Management of Health and Safety at Work Regulations efer to the legal requirement to have safe systems which accident investigation is included, Railway Group Standard GO/RT3119 mandates that persons who lead or manage the investigation process must have the competency to do so.
